I need to put a small 3 question survey on one of my landing pages. Two are essay questions and one is multiple choice question using radio buttons. I have an account with but they have the email address as a required on all their forms and I don't want to have that field. I also have used survey monkey but to imbed a small survey on my page seems very complicated. I just need to gather the survey answers on an excel spread sheet. I am not very IT savy but I can put basic HTML codes on my web page and do very basis editing of HTML codes.
