I can surely help you with the Salesforce implementation, including the sample tracking and the time based automatons. The customer survey part is not actually done in Salesforce, as some external services like Clicktools or FormAssembly may be needed for the job; even though both providers have a Salesforce integration in place, this part alone is an entire new project as they require a heavy configuration and the preparation of surveys and templates; some other even more expensive solutions for collecting data and run customer surveys with a solid Salesforce integration are also provided by Qualtrics.

I checked the attachment and it can be done without the standard objects by creating new objects such as customers and Customers feedback plus some workflows to automate the emails needed to be sent to the customer and delivery schedule.
