Create Scope, Requirements & Test Plan

I need for someone to develop a requirements document for my software project. My software project... generally... is entitled "Desktop Assistant" It sits in the taskbar and comes alive on a come-alive event.? It then asks for a customer's SSN or account number and executes an XML query against an SQL database. Based on the database's response, it will instruct the user to do something. For example, say customer 12345 has an eMail address that is out of date. The customer service rep will enter 12345 and Desktop Assistant will execute an XML request.? THe reply will indicate that an eMail address should be collected/verified. The eMail address is collected/verified... process over. It is much more complicated than that.... but this is the essence.
