Wordpress form builder multiple upload issue

We have a form builder in Wordpress, using which we're trying to email documents to users so they can sign them, or upload image, and we're trying to attach them to that user profile using shortcodes. If you have worked with form builder and shortcodes, please apply. Work starts now, and we need issue fixed asap.
