I need someone to edit/create bootstrap for my website

I'm trying to automate my sign up process on my website. The website is www.mowdayton.com. The form in question is [login to view URL] You can test the form out using address 2025 S Linda Drive, Bellbrook, OH 45305. You will see when you enter the address and click submit it comes up with a price for service. If you select a price it goes to a separate form. I want the same functionality as www.budgetlawncare.net. When you go to their page and click on the "See my price" button it opens a window. I want mine to function the same way theirs does. First window screen I want all the same fields. 2nd window shows the prices with options for weekly or bi-weekly with a price (mine will have 3 options, weekly, bi-weekly, and One time mow). There is also functionality to set your own day for service. I do not want that built in. Once you click on weekly or bi-weekly you scroll down to select more options and prices. In the left column will be Aeration and Weed control and fertilization. The right column will include Mulching, Flower bed maintenance, Bush trimming, Spring cleanup, leaf cleanup, snow removal. One column gives prices and check boxes to sign up. The other column gives other services that do not have prices with check boxes. The 3rd window has other information that needs filled out. I want all the same information in this window. The 4th window has the payment information. I want the same information. Once the form is submitted the customer should be sent to a thank you page. They should receive an automate email with that I can provide. I should be sent 2 emails, 1 with all the information they filled out on the first 3 windows of the form. The 2nd email sent to me should be all information from all 4 windows of the form. Currently, when a customer visits my website, the put in their address and click submit. It is currently programmed to pull a customers property size via [login to view URL] API. It compares the property size to a chart on my server that give is a price for weekly, bi-weekly and one time services. I will need this functionality on the 2nd window after a customer has clicked on weekly, bi-weekly, or one time mow for Aeration and Weed control and Fertilization.
