Need single webpage developed. Page is simple.  Just a questionnaire. Data written to SQL server. The webpage will be launched from another program. Using either HTTP URL or POST parameters the program having the link to the new webpage will be sending the unique identifiers for database. 1. The look should be like the included PDF. 2. The individual completing the form be can pick as many choices as they need in the question section. a. Any question with no response is a “no” b. Any question with a click button is a “yes” 3. The free text should have about 1000-character limit with a countdown of remaining characters. 4. The responses will be stored in SQL Server 2012 11.0.2100.60(x64) a. Yes or No response for each question b. CaseId – Use as Primary Key; Will come over in HTTP URL or POST Parameter c. CSN number – Will come over in HTTP URL or HOST Parameter d. DateTime submission e. SQL Server Table is existing and file is attached. 5. Subsequent questionnaires with the same CaseID can be filled out. a. Any “yes” should overwrite “no,” but “no” cannot overwrite “yes.” b. Any new comments need to be added to existing comments. I understand this may require another SQL table. 6. After hitting submit the page should change to a blank one saying, “Thank You.” And then the window should close after 1 second. 7. Vendor’s choice for HTTP request including the identifying information; URL Parameters or POST Parameters 8. Needs to be usable in IE version 11 9. I expect to make content changes in the future and will be using Dreamweaver or similar. 10. I will be hosting the website and any associated code on local servers. 11. Included documents: PDF of what page should look like, SQL Script of existing database, PDF describing the program that will be used to launch the website.
