Display global map with city, state, country data points added by PHPBB users

Description: -Goal is to track where users were located when they were when diagnosed with Kawasaki's Disease. -This application will present a map & pin-pointed graphical representation of that data. -Project will be hosted at http://www.kdforum.org. Requirements: -Display map image of the globe with "pins" w/shadows that represent what each PHPBB user has selected as their location at diagnosis. Must look nice. -User inputs: 1) person's first/last name, age -- textboxes 2) relationship -- textbox 3) location: city, state, zip, country -- text box, drop down, text box, drop down 4) season of the year -- drop down 5) date diagnosed 6) was person ill at the time, and if so, what was the illness. -- drop down, text box -The pin color will be one of four colors that represents what season the user has selected: spring = green, summer = yellow, fall = brown, or winter = blue. -Each user will be allowed multiple votes based on 1) each person that was diagnosed, and 2) date those people were diagnosed (one person can be diagnosed multiple times) -User data will be pulled from a PHPBB3 database, and this new application's data must be stored in a separate database.
