flash

hi, i need a copy of skinID flash evualation only, just the flash questionaire form not whole website. please review the flash skin evualation carefully. i will have series of flash questions and answers and i want all the information to be sent to me, there will be 4 to 5 questions which are really important to us and rest of all questions will be a drama but important to take user attention. i like how their graphic is setup dragging, slider, loading etc and want similiar to it. if any question please ask i need someone to start on it right away.
