Simple formatting changes to a single page of a Creloaded website

I need the following changes to my heavily modified creloaded site. This is a live site, the developer will perform the work offline, and I will provide you with all the necessary files. Before you get fully paid all work will be tested, everything is working perfectly fine as is, I just need this page to look more presentable. 1. Step 6 & 7 need to be reformatted to reflect the exampled shown. Currently the page looks like this [login to view URL] and it needs to look like this [login to view URL] please view this carefully, I expect it to look exactly like the sample given. 2. The “Add Comments About Your Order” was text box was moved from step 7 to step 6 and renamed “Special Delivery Instructions” 3. Instead of including the [login to view URL] in the [login to view URL] page I would like to use a separate header called [login to view URL] – I will provide you with this file. 4. On step 7 please comment out the coupon feature (do not delete)
