BOONEX DOLPHIN DEVELOPER REQUIRED

Hello. I have a custom development job for Dolphin. I need someone to start it right away and complete it fast. Full details can be found below. Allready Developed is an area of the site called THE VAULT which is password protected on everyones profile. It is an area that only can be viewed by other members IF they have the vault password. Inside this area we want to have ONE form, split into 4 sections that will contain very personal information. I have a full excel file with all questions. I will upload a picture of the first sheet. The Structure of each sheet is exactly the same - Therefore, you will need to create one form, then duplicate it 3 more times changing only the questions. If you look at the screenshot, there is 7 areas for each of the 4 sections. The user can input at will, personal information about themselves. For each area in which people can input information, there is also a number of questions on how things have effected them. This needs to be a simple Ticking of a box for Yes or No. In the screenshot you can see an example... I need this creating into an Online form. At the bottom of each section you need to be able to SAVE it. So you can do 1 section at a time. The full set of questions/answers should be able to be downloaded as a PDF by people who have access to The Vault. Please respond with a price and timescale, i would like to start as fast as today! ## Deliverables 1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done. 2) Deliverables must be in ready-to-run condition, as follows?
