Add fields to forms and MySQL queries on existing Zend / Doctrine application

Updates on the system are requested and the original coder is unavailable. The application is a balloting system and the focus of the updates is on a proxy form, including two checkboxes and four text input forms. Some existing logic (go back and fill out form completely) needs to be added per the new fields. The overall functionality of the application is completed, the new fields and logic need to be added to existing fields and logic, therefore the project should be fairly easy to follow for someone familiar with Zend / Doctrine. I can provide SSH access to the site, MySQL table definitions and detailed orientation to the requirements. I estimate that the work would take about two hours for someone with a solid background in Zend / Doctrine, in addition to any time needd for orientation.
