Write PHP page for aMember signup

I've had problems with my aMember site. Several of the problems I have just been able to correct, so am editing this Bid Request. One item remains:? Create a PHP page to fix current page which is not functional. ## Deliverables There were several problems on this site, but I have corrected all but one.? Therefore, I am editing this bid. Currently, here's the problem.? On the index page of the website, [[login to view URL]][1], there are 3 signup boxes on the left hand side of the page. In the 3rd box, which is for buyers of? my book, when visitors click on “Claim your free gift Silver membership,?? they are taken to the page to enter the code word.? Code word is “days.??? But when they put in code word, the page simply renews, rather than taking them to credit card information.? This page is written in PHP.? It worked for several weeks, then stopped working.? The page needs to be corrected or rewritten. Buyers of book should be taken to page where they can enter credit card information and receive 2 free months of Silver membership. Below I've included a zip file of the code from the existing page. This is an URGENT job.? This website is a backend support service for buyers of my book, and they currently cannot enroll in the membership site.? The site has been down for 2 weeks, while I've tried various solutions to the problem.? Therefore, I need someone who can do this immediately.? Thanks.
