Classy Opt-in Form Design with Pop-Up Function

I need an opt-in form designed using my [login to view URL] code, which I will provide you with. The form design needs to be classy and sexy. I am looking for something very similar to the one on this site: [login to view URL] My form does not need the VeriSign logo, everything else should be the same, including alignment, buttons, border, background color, size, etc. I will provide the text for the form intro ??" the first block of text. All attributes ??" colors, sizes, values, etc need to be editable, and when possible, include “hidden?? comments in the code explaining what each is for. This is so that the box can be modified as needed for various areas of the site. I also need this opt-in form scripted as a pop-up, using javascript. The pop-up needs to come up when the visitor first enters the page. The pop-up form needs to be scripted to that it only pops up ONCE per visit. For example, if someone comes to one internal page on the site, the pop-up would come up asking them if they want to opt in. When they click through to other pages on the site, the pop-up should not come up again. This should happen no matter what page they come into the site on. I need this script and pop up on every single page of the site, but it should only come up once per visitor. The right candidate will have experience with javascript and html. You will also have experience setting up and designing opt-in forms. Please provide samples of your work in this area. Proposals submitted without samples and past experience will not be considered.
