PDF FORM JAVA

We have a digital .PDF form that has been designed. It has a submit button via email for which java scipt has to be created so that it can be filled in by respondents using all platforms of email and adobe readers with the completed form provided in .PDF format to the sender ( See screen grab). ie eradicate the limitation with client-side software which is why it is not possible to use the mailto: syntax which varies from user to user. You will need to develop a custom java script and link it to the submit function to call a PHP/ASP script with variables in the URL and submit as = pdf. The script should receive the data in $HTTP_RAW_POST_DATA and save it into a temporary file and builds the mail function parameters to send mail with the attached pdf form. Please note we will not be submitting the form directly, so the final form needs to be emailed to our client for distribution. Please also allow for two author changes as well as indicate how long this will take.
