simple database

I want to create a database on my site that I can enter new information into. The database will have five fields and be sortable, and let me cross reference them. For example, look at: [login to view URL] I want to make this same database on my own site. I also need to be able to enter new types of information within the existing five fields. My site uses [login to view URL] webhosting, linux servers, mysql. All text to be in times new roman 11 point font. also need a search function so someone can type in what they are looking for and it will bring up all entries that match. I will provide an Excel sheet with the first 39,500 entries for the database. Also need the ability to enter basic html code on the page above the database section, so I can type in an explanation of what it is. ## Deliverables
