REPOST: Need a form on my wordpress site

BUDGET - $40 MAX NO MORE See attached PDF - Here is what I need done: 1. The attached PDF must be made into a form on our WordPress site with the exact same fields and text. 2. Above the Submit button there needs to be an email address field - this is where the form will be emailed to. The title for this field will be 'Enter your email address to get this form sent to you." 3. The form must be emailed as PDF. 4. The PDF must have a fixed header logo. I will give this to you. I will also show you a sample of what this must look like. The form must look exactly as this. 5. The form must also be emailed to another address that is fixed and will not change. This must happen automatically without needing to enter this in. 6. The text typed into the form must be stored in our WordPress database. I also need this database to be accessible in the backend of the website and it must be exportable to Excel. 7. I also need the Excel document to be automatically emailed to the same fixed email address as mentioned in point 5 above each time a form is submitted. Please use a WordPress form plugin for this (drag and drop is preferable) This is a quick easy job. I need this done today within the next few hours.
