need help coding an online test and results

I am looking to develop an online test which delivers instant results to the person filling in the test. The test is 10-20 questions long, with the answers corresponding to one of four answers, with a required tally at the end of the test. I am just getting the test completed and will be able to deliver the questions and answers very soon.
