Get Quote Calculator for a moving company in PHP

I need to add a Moving Quote calculator to one of my client's website written in PHP. The calculator should function exactly as it appears on this website: [login to view URL] (this site uses a WordPress plug-in for that functionality, which is no longer available). Please enter sample data on that site to find out the needed functionality. I'm attaching the list of items to be included into that calculator along with their cubic feet dimensions which are to be included into the calculation. I'm attaching the items list along with their cubic feet count in the Shipment Size [login to view URL] document. The results of the online quote should be nicely formatted with all the information filled out by the user (including their name, email address, phone number, from and to addresses for the move, and all the items they have selected in the calculator along with the square footage and totals and then sent to an email address.
