Form + Automated E-mail Script

I am looking for a developer to help create a simplistic but system that collects data through a web form and sends an automated e-mail to a set group of recipients. Here's how I would like it to work: 1) E-mail reminder automatically sent to set group of recipients each week containing a link to a web form 2) Recipients visit the web form once and for each day of the upcoming work week select which one of our two local company offices they plan to be in (or neither). 3) Each afternoon an automated e-mail is sent out to all recipients listing the names of the individuals who will be in each of the two offices on the following business day. 4) Restart at step 1 for next work week. Open to suggestions on the platform, but we currently host our website through hostgator using wordpress. Also have limesurvey deployed on the server and have a paid surveymonkey account if that is easier to use for the data collection. All recipients/employees also have Microsoft Exchange e-mail if there is an easier way to accomplish the same goal with e-mail rules, shared calendars and the like -- though I doubt it.
