Create a Webpage Form

I need a php google maps api expert to create a webpage form similar to Add a missing place of google maps functionality, replacing the current webpage form of my website about local search. The webpage is written in php, javascript, html5, using google maps api, and saving data to mongodb collection documents. The requirements of the webpage form: - similar to Add a missing place functionality of google maps, or better, but matching to our web theme color - using text variables for display so can be easily translated into other languages at later times - responsive design with mobile user priority (faster loading) - Place name autocomplete for nearby or similar names of places already existed, go to detail on click - Address autocomplete or position marker on map, fullscreen map when chosen - selectable options for every possible columns for easy use on mobile - add photo from galery / camera / file browser, save photo file to aws s3 and save link to the document - switched to edit mode if there is query parameter, and load existing data, modify on submission - get login status check using facebook login api, redirect to login page is not currently logged in - notify user whether submission is successful or failed
