Create online html layout from csv file

1. create an upload page for 1 file (CSV) 2. when click generate, then generate HTML layout from the information on the CSV file as specified on my layout. The layout should be set so I can print in 1 go. There is only I've included in the attached files an example of the CSV file (in xlsx format) (no need to save data, just generate the layout and I will print)

Hello, I have done the as similar app developed in C#. I know how to do this. There is no matter to apply in the HTML and Js. I'm gone through the attached layout form it's quite clear to do this. Thanks, Sheik
