Consultation about upcoming website and app project

I am paying you for consultation. I have a website which will have a form that needs to be developed. After the website is developed, an app will be created for both IOS and Android. I am looking for someone who can code: 1) Preferably in a WordPress setting 2) Create a website-based AJAX form which will have features to include - standard text fields - conditional fields - lookup field that will auto populate the form with previously submitted information - when submitted, it will assign the form a unique identifier (like a PO number) - a spreadsheet-like entry form that automatically adds another row if the customer needs to add another product - the form will ALSO be used in an app with both IOS and Google. - If the person leaves the form, it will retain their information - The form will have four screens with a progress bar 3) You ALSO need to be able to build the apps I want to know what you would charge me for the form portion of this website, with future incorporation into the apps.
