Need a 2 step form added to wordpress theme and info saved to MySQL table

Hello, I understand that you want a form similar to Cheeky Meet site. We will create a plugin which you can install and put the form will be usable in pages and post pages of the wordpress site. It will be a stand alone plugin, which you can use with any site or any theme. We will make it responsive. We will setup the plugin with your theme on your site so that it works perfectly. Data will be saved in a table in the database. There will be a page in admin area to manage this data. You can create, read, update, delete the items from the form from the wordpress admin.
