Create reservation form

I need a reservation form for my new touring business website. The form is basically a contact form, with the following fields: - Name and surname - Email - Number of people - Tour selector - Date The only detail is that I'd like the tour to be pre-selected based on the page from where the form page is accessed. On the website, each tour will have its own individual page with detailed information and a reservations button. The button will link to the form page and the tour should be automatically selected when landing on the form page (from a drop-down menu, I imagine). I have good knowledge of HTML and will be able to implement this on each individual page, so you'll not be doing that. Your job will be to provide me with the code and some basic explanation of how I can implement it.
