Web Based Data Entry and Survey

We need a MySql/PHP based website that allows 1) Registered Users to enter text. The text is stored in a database. 2) The system also allows the same registered users to rate the text entered by other users. The ratings are stored in a database. 3) The administrator should be able to generate certain reports from the database. The complete functionality is described in the Deliverables section. Please read the deliverables section carefully before entering a bid. ## Deliverables **Functionality** The system will work in two modes- a data collection mode and a survey mode. Initially, the system is set in a data collection mode. Users log in to a website and are asked to enter up to 10 (administrator configurable) submissions. A submission is made up of a title and up to 1024 characters (admin configurable) of text. Users may enter all submissions at once or can log out at any time and come back to complete the task. They may view and edit their submissions at any time during collection mode. The submissions are stored in a database along with the author identifier and date/time entered. At some point, the administrator switches the system to survey mode. Users log on and are now presented with the submission evaluation system. Each user is presented with 100 (admin configurable) submissions randomly selected from the database of submissions. Those presented should not contain any submissions authored by the user. The user evaluates each submission on a scale of 1 to 10 using radio buttons. (The scale is labeled "terrible" to "amazing".) As soon as a radio button is selected, the evaluation is recorded and the next submission is presented. The user is informed on each cycle how many submissions remain to be evaluated. At any time the user may view the list of submissions already evaluated and update/modify an evaluation. Reports required are: 1. table of submissions with title, author, average rating, max rating, min rating, standard deviation of rating, number of ratings recorded. 2. clicking on a submission title reveals this information plus the full text of the submission. 3. table of users with: number of submissions entered, number of submissions evaluated, average evaluation given by the user, average rating of all submissions authored by the user, the average value of the highest rated submission of the author. 4. clicking on a user reveals this information plus a list of the user's submissions (with the information in 1 above). 5. reports 1 and 3 may be exported in tab-delimited file format. The administrator must have the ability to change the mode of the system from data entry to evaluation, change the "administrator configurable" parameters above. The administrator must be able to create a "new instance" of the system with a unique URL and populate it with a list of usernames and passwords. The administrator must be able to export each of the Database tables in a tab delimited or csv format. The system will be hosted on a server with PHP4.3.x, MySql, Perl and a Cgi-bin directory. We expect the system to be relatively small scale, with fewer than 100 users at one time. **Screens** The system will have 4 screens 1. User Login- A form asking for user name and the password. Ability to change password. 2. User Data Entry- A form asking for submissions (title and text). Users should be able to preview, edit, or delete previously submitted ideas. 3. Submission Rating- A form that presents the user sequentially with submissions and a ten point radio-button scale for each submission. A "previous submission" button allows the user to go back in the sequence of submissions evaluated. The user may view all submissions evaluated and edit the values of the ratings. 4. Evaluations database- User ID, submission ID, rating **Other:** Client will provide 1024x50 pixel .gif graphic for header on each page and a plain text footer for each page. **Deliverables** 1. Fully functional, bug-free and documented source code. 2. System installed by the seller in the buyer's server environment (hosted by Hostway). 3. Installation must be completed by 23rd October, 2006. 4. All deliverables will be considered "work made for hire" under U.S. Copyright law. Buyer will receive exclusive and complete copyrights to all work purchased. (No GPL, GNU, 3rd party components, etc. unless all copyright ramifications are explained AND AGREED TO by the buyer on the site per the coder's Seller Legal Agreement). ## Platform Web Based, should work with IE and Firefox.
