A Simple Quiz application for iPad 2

I need a simple 5 question quiz application to be developed which will fetch data from a pool of 150 questions stored in the application. This will not be launched on AppStore and needs to be installed on multiple iPad device. To attempt the quiz the applicant will have to fill up a 4 field form and then continue to attempt the quiz. The quiz once attempted would show a Congratulations page if 4 or more questions are marked correct. If less than 4 questions are answered correct it would show a Sorry and Attempt again page. There should be an interface to retrieve the data captured by the form in the application
