Create Review Funnel for Reviews

Hello Webdevs, I need a Review Funnel for several local clients. Site has do be easy costumizable and reuploadable for several clients. What is a Review Funnel: I mean with review funnel that a customer of a restaurant gets an card where he gets a free drink if he gives out a review on the page [login to view URL](my hosting domain).com On the page he has two options. Option 1) Give a Bad review ( thumbs down ) and automaticly opens a contact form where he can write us his bad review that i can work on it with the client to improve its service but dont hurts his rating. Option 2) Gives us a Good review ( thumbs up) and automaticly opens a window where he can press on a Google Review Button (redirect to Google review Page) or Facebook Review Page, or Yelp or some other. I need to harvest the Data given. So a counter on Good and Bad Review Button hits would be nice! _____________________________________________________________________ There is already a site that offers exactly what I need named "[login to view URL]" but it is to expensive for several usage and several clients.
