Hi, I have experience with implantable antenna design, for example, this antennas are inmerse on tissue with losses and we can add that this antennas are small, when the antennas are small trent to have small gain and if the surround media is lossing it make too that the gain low. If you can share more details with me. Commonly you can meet the S11 requirements but for enhance the gain you must that to applicate techniques like the use of complementary structures or augment the phisical size of the antenna. we can discuss more detais in chat.
