i checked your file, it just one thing which little bit complex and thats is check boxes for checkboxes, we need macro which work on it otherwise, check boxes never work in word document i already completed too many projects of this i can convert this file in word/excel which you prefer while, near me, excel is better than word anyhow, whatever your preference, let me know i can complete it within hours, not days looking your response thx
