A small change to a PHP program

Go to [[login to view URL]][1] -- Note the long sign-up Form the Left site. I need to make some minor changes to this form, such as below, Itme number 1 1. Change the text size to 14 point. 2. Change "Full Name" to "Last Name First - First Name Last." 3. Change "Password" to "Your Prefered Password." (add the same "Check Availability" you can now see under "Choose a Username:" Then we can "Check availability" of (and search) both the User Name and Password. 4. Add Two "Open [login to view URL] Databases" that will contain the following: 1. This form that customers fill out. Administrator must be able to do a search of every item in the database. 2. Track all "Sign In's" giving the Username, and time of sign-in. And if possible, give us the IP of the "sign-in." 5. We want to use all Open [login to view URL] products that we can on this web site. 6. Give the administrator the ability to search, remove, or add new items to both databases. Give me two prices. One for the above Item, Number 1 and one for the following Item, Number 2. Iteme Number 2 Put a "link" in each Users "private site" that will open up a new "web site." There must also be a link in the new "web site" that will send the "User" back to his "[[login to view URL]][1] site."
