Custom Data Capture - integrate with WordPress

Word Press Data Capture Background I am using WordPress as a framework for a practice management tool for a law firm. In an effort to reduce data capture down to a minimum, and thus improve data integrity, I need a form/questionnaire developed to handle ‘matter inception’. i.e. the creation of a new piece of work for a client. Overview A matter consists at its most basic of a client ID, matter title, matter description and a work type code. This is largely coded and the coder will be required to extend on the existing code for this questionnaire. If the work-type is debt recovery, we need to capture a lot more information and it is here that the majority of this piece of coding comes into play. see attached zip file for more information ## Deliverables 1) Complete and fully-functional working script in source code form together with sql schema 2) source code to be commented in accordance with phpdoc standards 3) All deliverables will be considered "work made for hire" under U.S. Copyright law.
