Custom extension to be written

I need a CMS plugin to be written where I am able to manage 'profiles' for different people within my organization. It is easier if I try to explain what the end result will be. A new sales rep joins my team, I want to then go into the wordpress plug-in page, fill in a form with their details, first name(s), last name(s), telephone number, mobile number, photograph of them, e-mail address, town and postcode. I will also need to be able to choose whether they are a Contact Manager user (in which case, a custom form action will be provided), or not, in which case, the capture page data will be sent via e-mail to the sales rep. I want that to then generate a 'page/post' that will look the same as every other - a capture page, that will collect visitors' details. I want to be able to manage these pages from the plug-in, so if a rep leaves, I can delete their 'page'. The URL should look something like - [login to view URL] ([login to view URL])
