Custom Google Map Contact Form

If you can do it sooner then please let me know as you will be given priority. I need a Google map contact form that works in the following way. 1. User types in there UK postcode 2. They see a google map (Ariel View) 3. They drop a pin on there house roof (ariel view) 4. They fill in there contact details ie. Name, email address 5. We receive an email with a link to the google map showing the pin on there house and the other contact details.
