Add Submit File Functionality to Google Form Google App Script

What would it take to add a button at the end of the google form allowing the user to upload a pdf of their cover letter/resume (pdfs only)? [login to view URL] (Link above is a sample of the form. Link above does not have the script.) Would it be possible to then combine the pdf generated by the app form (03 - FS Profile Findings) with the pdf uploaded by the user?
