Advanced Poll Error

I added another domain to my server so instead of having the files for my old site in the main directory I created two sub folders, one for each site / domain. The problem is now the PHP poll on my main page ([login to view URL]) is giving out an error message and not working. I am sure there is a super easy solution and it will probably take someone who knows what they are doing about 2 minutes to fix...
