Asterisk based patient reminder service for clinic

I would like to have someone build an asterisk based api that will do patient calls reminder for multiple clinics. What I am looking for is doctor will have to login into a web portal from where enter customer details and he will submit patients appointment time. and then the system will make the call before 5 hours before appointment time. if customer is unable to answer the phone then the system will keep trying calling him after every 1 hour until his appointment time. Once patient get the call he will be informed about their appointment date & time and he will be asked to press 1 Confirm the appointment press 2 to reschedule your appointment press 3 to remind you before 1 hour of appointment time press 0 to cancel your appointment if patient will press 2 his call will be diverted to doctor clinic phone number. if he press 0 to cancel his appointment system will call on doctor clinic phone number to inform that patient ABC has canceled the appointment. Also doctor should be able to see the web based reports how many appointments has been made, how many calls has been made and what are the status of those calls. Plus call reminder time should be adjust able i mean doctor can select either he want to inform appointment time before 5 hours or 8 hours. how many time system should keep calling patient if he is unable to answer the phone etc
