Checkbox HTML Code - Output Takes User To URL Using Checkbox Values - Web Site Page/ Product Filter/Search Function

Dear Sir/Madam I want to use multiple checkboxes to output a URL I have attached an example of the check box menu. The user should be able to select one or all of the checkboxes. When the user clicks "fetch" they should be taken to the following URL [login to view URL] So if one checkbox is selected only 1 check box name is in the URL. If 20 checkboxes are selected then 20 check box names should be in the URL. Basically I am looking for a template which I can go on to customise (looks, titles,values) and extend (to include more options). If the template could be the same as the attached image. ie 6 rows with 3 options each. I will use CSS to deal with the styling myself.
