Change index page and create a search page in Yii using bootstrap

Only have budget for $40. NO Agencies or Companies. This is a small budget project and I want it to be cost effective. Experienced developers with no or little feedback welcomed :) Step 1. Need to create attached image (view using [login to view URL]) for index page. Need to use same or similar signin form as twitter.com. Invalid password warning should appear below login form. Footer at bottom and text at top left corner. Step 2. Need to create using Yii-bootstrap a search page based on all data contained in word doc 'SearchTabLayout'. There are several hidden sections so ajax will need to be used. At this stage, you will not need to link fields to database- just design layout and front end appearance. There will need to be space at bottom of page to display results. Skype discussion on it all will help you understand better. Thanks
