Convert a Word form to HTML5 form

I have an enrolment form in a Word document which I need to put online. Requirements: Phase 1: 1. All answers should be: a. Submitted to a google spreadsheet or similar real time system b. Each respond should be emailed to a list of email addresses 2. Put all questions online. 3. Some questions are required 4. Some questions would trigger other questions 5. Validations: a. Email address should be validated b. Phone number should be validated The form has roughly 40 questions. Phase 2: 1. Verification: a. Email should be verified by sending a confirmation email with a unique link. i. I should be able to change the verification email quickly and easily with a simple HTML editor b.
