Contact us form - Bottom of Page

I need to add a small contact us page to the bottom of 2 pages on my website... The contact us has to include 2 fields: Name: _____________ Email: ____________ *Submit* The form has to be placed inside an iframe and not to refresh page when form is submitted. When *Submit* button is clicked present rotating pre-loader for a few seconds and then "Thank You" message and bring form back. ## Deliverables Programmer should provide a code that I will be able to insert to the bottom of my page to create this simple form.
