create program in C# ( visual studio 2008)

Hello, I want to create a windows form application-( in visual studio 2008-in c#-) 1.) create a form to enter customers name, address,cell phone and date hired, employer name, employer address, and work phone number. ( I want the phone numbers to be in (201)-545-4545 format). 2. I want a preview form to display the entered data. 3. I need the header on each form to say customer data 4. lastly if the date hired is newer than the present date - there should be an error come up. thank you - ( I can pay 30 dollars)
