Android and Apple App that launches a URL only

We need an Android and iOS App Icon, the artwork will be supplied by us in .PNG format, that downloads from either our website or yours. We will supply the one page website design. see [login to view URL] for the webpage we have. The App is a demo App only not for public download and must be able to be downloaded from either your website or ours (See attached PDF) NOT the Itunes or Google Play store. You will need an Enterprise Developer Account for the iOS work. If you do not have the Enterprise Developer Account, you will not be able to do the iOS App.
