Digital forms- Medical forms for Android / iPhone

I have medical forms in pdf that needs to be converted or rebuilt to work on Android ( Samsung Galaxy tablet ) or iPad / iPhone. The forms must be fillable and must be able to accept signature WITHOUT the need to use any EXTRA APP. See the link below for the company that does this kind of job. I know it can be done. Fills out the form. Customer signs the form digitally. User can then e-mail form or upload it in the cloud.
