My little business collects leads via Wufoo forms embedded in our website.
Typically the website visitor follows a link from Adwords, Facebook Ads or email to a landing page on our website, then clicks to the enquiry form page (in which a Wufoo form is embedded) and sends an enquiry.
I want that enquiry form to contain fields for Source, Medium and Campaign which are to be prefilled from the UTM codes.
I have set up a demo landing page and enquiry page so you can see the set up at:

Wufoo has linked to a blog page from 2008 showing how to do this (using a previous Google Analytics system):

I need to be able to replicate your solution on several pages of our website, including pages we will add in future.
What I want you to do is:
- create a system for adding UTM codes to this form. Wufoo has a free plan and Squarespace has a free trial so you should be able to test this at no cost other than time.
- give me the log in to your demo Wufoo and Squarespace accounts so I can see how it works.
- give me idiot proof instructions as to how to make this work. This can be in an email or on the Squarespace site if that is easier.

I can either create an excel sheet for you that automatically populates the URL with all the UTM codes or come up with something different and more convenient for you. One of the most important things about tracking is being consistent and using the same labels across different channels.
