Advance JQuery designer needed for some UI effects

Need an expert JQuery designer to design this ([login to view URL]) UI for us. You dont have to use those peices of code however the end result should be similar. PLEASE NOTE: I only need the "Make a Quiz" part done, not the entire thing, at least not now - but could be a separate requirement later on with a different budget. We need the complete HTML with the embedded JQuery code (non-encoded format) and the CSS - proper documentation expected wherever applicable. This is an urgent requirement, so only serious people who can start on an immediate basis need apply. I am looking to pay between 50-80$ for this. We will not be escrowing or making any part payments. Once the job is completed satisfactorily, you will be given a full payment the very same day after handing us the deliverables and we'll leave you an excellent feedback.
