Blue pop up on bottom of screen

we want this blue pop up under the screen to pop up and always stay on the bottom of the screen if he page is scrolled down x will close the box after they enter in the email address and hit submit it should 1) email the person with our template, 2) enter email in db 3) then the box will change to thank you it should verify email
