convert logo for printing on transparent film

I have a logo that will be printed on transparent film. The formats I have are JPG, AI, PSD and EPS. I need two files created: 2 copies of the logo in a print area of 4.5" x 4.5" 1 copy of the logo in a print area of 19" x 12.35" Please provide one example of converting a print file for printing on with a transparent background. I expect to pay no more than $30 USD for this small task.
