Create Dynamic Web Form

Hello, We currently have a form that we use for some of our clients. We need to make some changes to it as well as give it a bit of a facelift and make it 2.0 looking/make it work smoother. The form would walk the user through some questions at which the form "dynamically" updates based on their answers. The form is then submitted and an email is sent off formatted in a certain way. This is phase 1 of our 3 phase solution so hopefully if this works well we can continue working with you for the additional phases. I can discuss the additional phases after bidding is complete but you are not under any obligation to do anything besides what is requested here.
