ASP Script for simple password only login

Need a simple login page that only requires a password that I can easily change in a config file. It needs to be a field that I can easily manage and build into a page similar to the attached image. I also need the field name to appear slightly faded and disappear when the user starts typing. Also, i need the ability to define two images for the submit button so it rolls-over to a different image when moused over.
