Create a Civicrm donate page

Looking to have a civicrm donation page made that looks and works like the EFF donate page [url removed, login to view] The look and information (on the right) needs to be changed without too much difficulty (ideally using blocks, but is fine if hard coded). The key features is the levels being able to be the same and then be recurring with a single click. The template must also support the enter your own amount option included in the civi form. Having the pay with credit card and paypal buttons bring up the credit card form or go to paypal should work (its just simple JS as its all on the page already with standard civi install).
Basically it is a pretty standard civi template with some JS skill required. Shouldn't take more than a few hours someone as standard civi has most of this stuff built into the default templates.
Drupal Version is 6 and Civicrm version is 4.4.5 (4.5 the newest version is fine too).
